Registration

Registration Form

We are registering for the 2024-2025 school year starting on December 1st, 2023. All families should register on-line. New family/non-church member registrations will be held and processed after January 8th, 2024.

Required Forms

The following forms are required by Ohio State Law and must be completed by August 1, 2024. All lines on both forms must be completed (even if information is duplicated).

• New Medical Form

• New Enrollment Form

• Care Plan (as needed)

• Medication Form (as needed)

Optional Form

The Family and Social History form is not required by law; however, our teachers find it very useful and we encourage you to complete it.
