Registration

Registration Form

We are registering for the 2026-2027 school year starting on December 1st, 2025. All families should register on-line. New families/non- church member registration will begin December 15th.

Required Forms

The following forms are required by Ohio State Law and must be completed by August 1, 2026. All lines on both forms must be completed (even if information is duplicated).

• New Medical Form

• New Enrollment Form

• Care Plan (as needed)

• Medication Form (as needed)

• Family and Social History Form
